Limited Engagement: You understand that this is a limited engagement with the Clinic for the purpose of providing weight loss services via telemedicine, and the Clinic has no obligation to treat you for any other conditions discovered or discussed during the consultation.
No Emergency Services: Telemedicine services are not for medical emergencies. For emergencies, you must call 911 or go to the nearest emergency room.
Not a Substitute for In-Person Care: You understand that a telemedicine visit may not be sufficient for your medical needs. You may be required to seek additional in-person medical care, and you agree to do so if advised by a provider or if your condition persists.

Privacy and Confidentiality: You are responsible for ensuring your own privacy during virtual visits. You must make measures to prevent unauthorized access to your information, such as ensuring you are in a private location and disabling any virtual assistant devices. You understand that you waive confidentiality if your personal health information is overheard by another individual.
Data Security: You acknowledge and consent to the recording and processing of your personal and medical data for the purposes of treatment and service delivery. We make reasonable efforts to protect your data, but you understand that no electronic transmission is completely secure and there may be risks associated with the technology used.
